NTSB stated that the Tesla Model 3’s driver operated the car with Autopilot on, but was pushing the accelerator pedal manually. The vehicle entered override mode after striking a van at the back. Both the Tesla’s driver and passenger were then fatally injured when it struck another truck at high speed.
