In the midst of uncertainty as to whether the more contagious omicron virus will spread in America, the Food and Drug Administration approved a fourth Pfizer Covid vaccination dose.

Pfizer released a statement Tuesday stating that the FDA had also approved a booster shot for patients 12 years and over who have compromised immune system. Moderna should receive the FDA’s similar authorization this week.

FDA took the decision to approve the vaccine without meeting its advisory committee. This is a rare step that FDA has taken more often in the past few years as it tries to increase the use of Covid vaccines. Just two weeks later, the FDA’s approval comes from the drug regulator. Pfizer ModernaThe FDA was asked to allow a second booster shot, based upon data from Israel. It is likely that the Centers for Disease Control and Prevention will quickly approve of this decision.

This FDA decision bypasses the advisory committee on vaccinations that was due to meet April 6, to discuss future booster shots in America. Expect a general discussion among vaccine experts about boosters but no vote.

A member of the committee, Dr. Paul Offit, criticised the FDA for not holding an open meeting so that the public could hear the experts weigh the data, and then make recommendations to FDA regarding the best way forward. Although the recommendations of the vaccine advisory committee are not binding, they offer transparency to the public.

Offit spoke out about the FDA approval, saying that it was just “sort of fait accompli”. So is that how it works? We talk endlessly about how  we follow the science — it doesn’t seem to work out that way.”

Since last fall, the FDA hasn’t asked committee members for a recommendation regarding booster shots. they voted in favor of a third Pfizer or Moderna doseFor people 65 years and over and for younger persons at high risk of severe Covid.

Experts in health disagree about whether an additional vaccine is necessary right now. However, the argument is complex. A wider consensus exists that elderly and immune-compromised individuals may be able to benefit from extra protection. A booster shot is also available for healthy young adults, who are more susceptible to serious illness due to Covid.

Baylor College of Medicine’s vaccine expert, Dr. Peter Hotez said that some scientists think the shot is only for keeping people from the hospital. Hotez stated that vaccine policies should aim to avoid infection as well as long-term Covid. Hotez said the effectiveness of the third vaccine against hospitalization by omicron also has declined in recent years.

The CDC published a study in FebruaryIt was found that the efficacy of the third dose for emergency room visits decreased from 87% – 66%, to 91% – 78% to hospitalizations at 4 months.

Hotez said, “That makes me worry that boosters may not be holding up as well” and strongly supported a fourth dose. Hotez cited Israeli data that showed another booster improves protection for older people over 60.

Israeli scientists in a study published last weeK found that the death rate due to omicron was 78% less in seniors 60-100 years old who had received a fourth. PfizerThe dose received was lower than those who were given just three shots. This study was not peer reviewed and examined more than 500,000 medical records from Clalit Health Services in Israel, Israel’s biggest health-care provider.

Moderna was not satisfied with the FDA’s authorization of fourth doses to be administered for patients 50+. The FDA approval granted them four additional doses. Pfizer requested the FDA to allow fourth doses to be cleared for individuals 65 or over. Moderna, however, asked that they be allowed for adults 18 years old and above. Moderna CEO Stephane Bancel told CNBC last weekBiotech companies filed a longer request with FDA in an effort to grant FDA more flexibility when deciding which age group requires a fourth dose.

John Moore, an immunology professor at Weill Cornell Medical College, criticised Moderna’s request to give a fourth dose to all adults. It was an aggressive blanket application that didn’t differentiate between the needs of older and younger individuals.

Moore explained that an elderly or frail individual needs a higher dose of vaccine than a young and healthy athlete.

Fourth shots are now allowed because a contagious subvariant of omicron, BA.2, is spreading rapidly in Europe and China. This has been the worst epidemic since 2020. BA.2 is now the most popular variant in America and has been since February. White House Chief Medical Advisor Dr. Anthony Fauci said that infections could increase in the U.S. because of BA.2. he doesn’t expect another wave.

BA.2 does not generally make people more sick than BA.1; the vaccines are effective against both variants of BA.2 according to research from South Africa and Qatar. Both studies have not undergone peer-review.

As there is less evidence to support such an approval, it’s not clear if FDA will allow fourth doses to be approved for older adults. An Israeli study from FebruaryA majority of health care workers aged 18 or older found that the fourth dose doesn’t boost immunity, but does restore it to its peak strength. The fourth dose did not prevent infection in many people, but they experienced mild to no symptoms.

“A fourth vaccine of healthy young workers in health care may not have any marginal benefits,” said Dr. Gili RegevYochay, a Sheba Medical Center scientist and an Israeli Ministry of Health team member who wrote this letter to the New England Journal of Medicine.

In September 2013, the FDA advisory panel voted against any third dose of glycerin for all people 16 years and over. Experts felt that there was not enough evidence to justify such a move. Two months later the FDA reaffirmed its decision. authorized Moderna and Pfizer boostersfor adults who have not met with an advisory panel. After that, eligibility was lowered to 12/12 years.
